Senior House Staff Survey Regarding Post-Acute Care Training
Introduction/Objective: Forty percent of hospitalized Medicare patients need post-acute care (PAC). PAC is delivered in skilled nursing homes, acute rehabilitation facilities and at home. Selecting the right PAC option is a critical piece of transitional care. Unfortunately, current house staff seem to lack the training to assist the patients in selecting the most appropriate PAC venue.
